Who Needs a DOT Physical?
You’re required to pass a DOT physical exam if you:
• Has a gross vehicle weight or weight rating, or a gross combination vehicle weight or weight rating, of 10,001 pounds or more;
• Is designed or used to transport more than 8 passengers (including the driver) for compensation;
• Is designed or used to transport more than 15 passengers (including the driver) not for compensation; or
• Transports hazardous materials in quantities that require placarding under the hazardous materials regulations.

What’s Included in a DOT Physical?

- A complete list of current medications and dosages
- Names and addresses of your doctors
- Glasses, contacts, or hearing aids if you use them
- Recent blood sugar logs and A1C results (for diabetes)
- CPAP usage data from the past 90 days (for sleep apnea)
- Letters from specialists if you have heart or neurological conditions
- Recent bloodwork and clearance (if on blood thinners)
- Documentation if you’ve experienced limb loss or use medications that cause drowsiness
